Residential & Supervised Living Services
• 24-hour supervised living in a licensed group home setting
• Safe, structured, and supportive home environment
• Assistance with daily routines and household living skills
In-Home Support Services (ISS)
• An ISS Center is offered through our agency, based on authorization and eligibility
Personal Care & Daily Living Support
• Assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and feeding
• Total care support for individuals with complex medical or mobility needs
• Skilled nursing services as authorized
• Health assessments and ongoing monitoring
• Coordination with physicians and healthcare providers
• Support for individuals with complex medical needs
Medication & Health Monitoring
• Medication administration and monitoring per physician orders
• Ongoing health and safety monitoring, including observation and documentation
• Coordination of routine and specialized dental care
• Assistance with scheduling and attending dental appointments
• Support to ensure dental services are accessed safely and consistently
Behavioral & Emotional Support
• Implementation of behavior support strategies outlined in individual care plans
• Support for individuals with communication challenges, including non-verbal individuals
• Crisis prevention, de-escalation, and emotional regulation support
Community Integration & Life Skills
• Support with community outings and activities
• Social skill development and meaningful engagement
• Assistance with daily structure, routines, and personal goals
Transportation & Appointment Coordination
• Assistance with scheduling and attending medical, dental, and therapy appointments
• Coordination of Medicaid or approved transportation services
Individualized Care Planning
• Person-centered planning based on individual needs, preferences, and goals
• Collaboration with families, guardians, case managers, and interdisciplinary teams
• Ongoing documentation and compliance with HCS and state regulations
